REQUIRED FORMS

Before being allowed to practice, you must have the following completed and on record:

1. Medical Exam/Physician Physical Form

2. Parent/Guardian Permission Form and Emergency Information

3. Warning Letter to Parents and Students

4. Online Athlete Information Form (click here)

5. $60 Athletic Fee to the GW Treasurer, Mrs. Weightman. (Make checks payable to: George Washington High School) must be paid prior to any participation with the team.  (All checks must have the Colorado Driver's License number of the person signing the check and the DPS ID number of the student.)

6. $50 Equipment Fee (make checks payable to Dana Johnston) must be paid prior to any participation with the team.  Optional hoodie is available this year.  Click here for the TEAM GEAR ORDER FORM.

7. If you are an athlete that will also be competing for a club team while participating in a sport at GW, you must complete and have on file with the athletic director an Outside Practice Permission form.  Turn this form in to Coach Mark.
